Clear Aligner Treatment (Invisalign): Everything You Need to Know About Invisible Orthodontics

We meet many adults who have ignored their crowded teeth for years because they did not want to wear braces, who cover their mouth with their hand when smiling, and who press their lips together in photographs. Clear aligner treatment is a modern orthodontic approach designed precisely for these patients. Without metal brackets and wires, it aligns the teeth gradually with removable and almost invisible aligners.

Demand for clear aligners has risen sharply in Turkey over the last five years. At our boutique clinic Livera Clinic in Florya, the vast majority of patients now ask for this option instead of traditional braces. In this guide we explain in detail how the treatment works, in which cases it really delivers results, the differences between Invisalign and other brands, and how the process is run inside our clinic.

What Is Clear Aligner Treatment?

A clear aligner, also known as an invisible brace, is a thin and flexible orthodontic appliance made from medical-grade thermoplastic polymer. These custom-made aligners move the teeth toward a planned target with controlled forces. Each aligner produces a small movement, and the next aligner moves things one step further. By the end of treatment, the dozens of aligners worn in sequence have brought the teeth into the desired position.

The system was first launched in 1999 by Align Technology under the Invisalign brand. Today there are dozens of clear aligner brands worldwide. In Turkey, many local and imported systems are used in clinics. The differences between brands stem from factors such as the quality of the aligner material, the precision of the software, the production technology and the clinical support.

How Do Clear Aligners Work?

The logic looks simple, but there is serious digital engineering behind it. First, a digital model of the teeth is created with millimetric precision using an intraoral 3D scanner (such as iTero). Then the starting position and the target position of the teeth are defined in the orthodontic software. The software divides the movement between these two points into steps of 0.1 to 0.3 millimetres. Each step corresponds to one aligner.

When the aligners are placed over the teeth, they apply gentle pressure. This pressure stimulates the periodontal ligament surrounding the tooth root and triggers the bone remodelling process. Old bone tissue slowly resorbs, and new bone forms at the target position. The tooth moves gradually within physiological limits. Because the movement is very slow and controlled, the pain level is significantly lower than with traditional braces.

What Are Attachments and IPR?

Some movements are difficult to achieve with a flat aligner. For rotation, extrusion (pulling the tooth downward) or root-oriented movements, small composite bumps (attachments) are bonded to the tooth surface. These act as the grip points of the aligner on the tooth and are almost invisible from the outside. IPR stands for interproximal reduction, the enamel reshaping of one tenth of a millimetre between crowded teeth. This creates space for the tooth and resolves crowding without extractions.

Clear Aligners vs. Traditional Braces (Brackets)

Both methods achieve the goal of orthodontic treatment, but the patient experience is completely different. The points below summarise the factors that really matter when making your decision.

Aesthetics

When a clear aligner is in place, it is not noticeable from 1 to 2 metres away. For professionals who meet clients face to face, work on stage or appear on camera, this difference is significant. Metal brackets, on the other hand, are clearly visible both when smiling and in close-up shots. Even though intermediate options such as clear brackets or lingual braces exist, they cannot compete with aligners on aesthetics.

Comfort

Metal brackets cause ulcers, sores and mucosal irritation inside the lips and cheeks. When a wire end pokes out, urgent intervention may be needed. Clear aligners, with their soft rounded edges, do not damage the soft tissues. Even so, a feeling of pressure during the first 24 to 48 hours of a new aligner is normal.

Oral Hygiene

With bracket treatment, brushing becomes difficult, and the risk of plaque build-up and gum inflammation rises. White spots (demineralisation) may appear on the tooth surfaces at the end of treatment. Because clear aligners can be removed, the normal brushing and flossing routine continues. This is a critical advantage for long-term dental health.

Eating

With brackets, hard, sticky or very hot foods are avoided. With aligners, you can remove the aligner and eat anything. However, you must brush your teeth after eating and put the aligner back in.

Check-Up Frequency

Traditional brace treatment requires an adjustment visit every 3 to 4 weeks. With clear aligners, check-ups can be spaced out to every 6 to 8 weeks, which is a major advantage for international patients.

Control in Complex Cases

Here, brackets can be one step ahead. In serious skeletal discrepancies, impacted teeth or very advanced rotations, fixed wire systems still deliver more predictable results in some scenarios. Even so, modern Invisalign protocols (such as mandibular advancement and gummy smile correction) close this gap further every year.

Which Cases Are Suitable for Clear Aligners?

There is a common misconception in the industry: some patients assume "I am a complex case, aligners will not work for me." In reality, modern clear aligner systems can address a wide range of issues, from mild aesthetic corrections to very broad orthodontic problems.

Mild Crowding

Rotation or overlap of 1 to 3 mm in the lower or upper incisors. Usually resolved in 6 to 9 months. This is the case type with the fastest results.

Diastema (Gaps Between Teeth)

Closing millimetric gaps, especially between the two front teeth. Aligners are highly successful here. A typical timeline is 4 to 8 months.

Deep Bite

The upper incisors cover the lower incisors too much. Controlled intrusion (pushing the tooth toward the gum) is achieved with aligners. This is a medium-difficulty case.

Open Bite

The front teeth do not touch when closed. Aligners are particularly effective in dental open bites. Severe skeletal cases may require surgical support.

Cross Bite

The upper and lower teeth close in a misaligned position. Aligners deliver good results in mild to moderate dental cross-bite cases.

Orthodontic Relapse

Adults who wore braces in childhood but did not use retainers, so their teeth shifted back. In this group, clear aligners produce very fast results, because the teeth have already been moved before.

Differences Between Child and Adult Treatments

Although adult patients make up the majority of aligner cases, there are protocols adapted for children and teenagers as well. Invisalign First is a version developed for children (ages 6 to 10, mixed dentition period). Invisalign Teen contains compensation features that adapt to teeth that are still erupting in teenagers. Because growth and development continue in children, the clinical schedule requires more frequent adjustments. In adults, although tooth movement is slightly slower because the bone is more stable, predictability is high.

Invisalign or Alternative Brands?

Patients often ask: "Is there a real difference between Invisalign and the other brands?" The answer is yes, but perhaps not in the way you think.

Invisalign

The most established brand on the market. Its software (ClinCheck) has the most advanced database. Its patented aligner material called SmartTrack applies more consistent forces. It has specific techniques such as Mandibular Advancement, Precision Wing and Optimized Attachment. The cost is higher than other brands, but it is the most reliable choice in complex cases.

Spark

Made by the Ormco company. The aligner material is clearer and less prone to staining (an advantage for coffee and tea drinkers). Comfort is rated as high. It is a strong alternative to Invisalign in mild to moderate cases.

ClearCorrect

A product of the Straumann group. Widely used in Europe and offers a price advantage. Thanks to its blue indicator series, the time the aligner has been worn can be tracked.

Suresmile Aligner

Dentsply Sirona's system. As it offers an in-house production option, it provides fast revision advantages in some cases.

Local and Regional Brands

There are also more affordable local systems developed in Turkey. They can be used in mild and selected cases. Successful results are possible with clinical experience and the correct indication, but predictability in complex cases may not match that of global brands.

At Livera Clinic, Dr. Ervin Umer scans the patient's case and creates a digital simulation, then discusses with you which system is most appropriate. We may recommend Invisalign for cases that require complex orthodontics, and more economical alternatives for mild aesthetic corrections.

The Treatment Process Step by Step

1. Initial Consultation and Clinical Examination

Gum health, cavities, old fillings, crowns and implant status are checked. Issues that need to be treated before orthodontics (such as decay) are handled first. Clear aligners begin in a healthy mouth.

2. 3D iTero Scan and X-Ray

Instead of a traditional impression tray, a digital model of the teeth is created in just a few minutes with the iTero intraoral scanner. No nausea, only a light sensation. Skeletal structure is analysed with panoramic X-ray and cephalometric film.

3. Digital Treatment Plan

The movements of the teeth are designed as an animation in the software. The patient also watches this animation and sees the end of treatment in advance. The ClinCheck simulation is often the decisive moment for patients when deciding to proceed.

4. Manufacturing the Aligners

The approved plan is sent to the production centre. Depending on the brand, production time varies between 2 and 5 weeks. The aligners usually arrive at the clinic as a sequentially numbered kit.

5. Bonding the Attachments

Before treatment begins, small composite bumps are bonded to the surfaces of the necessary teeth. This procedure is painless and takes 30 to 45 minutes.

6. Wearing the Aligners

Each aligner is worn for an average of 7 to 14 days. The daily wear time is 20 to 22 hours. Aligners are not removed from the mouth except for eating, brushing and special occasions.

7. Periodic Check-Ups

Check-ups are scheduled every 6 to 8 weeks. Online check-up options are also available for international patients. Through systems that allow iTero-like home scanning via mobile phone, the doctor can monitor remotely.

8. Refinement Phase

At the end of treatment, 95 to 98 percent of the teeth are in their planned position. For the remaining fine-tuning, 5 to 15 additional aligners (refinement) can be produced. This is usually already included in most packages.

9. Retention (Retainer) Phase

After treatment ends, the teeth must be held permanently in their new position. Otherwise, relapse happens very quickly. For retention, a clear aligner worn at night, a thin wire bonded behind the lower teeth or a combination of both is recommended. This phase is a lifelong part of treatment and must not be skipped.

Wearing Time and Discipline

The most critical rule of clear aligner treatment is very simple: they must be worn 20 to 22 hours a day. If this discipline is not maintained, the plan does not work. If a patient wears them only 12 hours a day and carries them in a bag the rest of the time, the teeth will not reach their target position. They can even shift back to their old position and double the treatment time.

In practice, the routine is: in the morning the aligner is removed for breakfast, the teeth are brushed after breakfast, and the aligner is put back in. During the day, the aligner is removed when drinking anything other than water. Dinner is the same. The total removed time should not exceed 2 to 4 hours per day.

The Livera Clinic Difference

At our boutique clinic in Florya, clear aligner treatment is carried out on the basis of one-to-one follow-up, without pushing the patient through an industrial flow. We would like to highlight a few points:

  • 3D iTero scanning: The most up-to-date intraoral scanner is used. The patient watches the end-of-treatment simulation on screen from the very first session.
  • In-clinic planning: The treatment plan is revised inside the clinic together with Dr. Ervin Umer, not in some assembly room. The software's suggestion is not the final word.
  • Boutique capacity: Because we see a limited number of patients per day, appointment times are long and your questions are answered quickly.
  • 5-star hotel and VIP transfer for international patients: Patients coming to Istanbul complete consultation, scanning and, if needed, the start of treatment in a single trip.
  • Multi-brand options: We offer not only Invisalign but also alternative systems suited to the case. The patient's budget and clinical needs are evaluated together.
  • Integrated planning with aesthetic treatments: For patients considering whitening, porcelain veneers or Hollywood Smile after clear aligners, the sequence is planned in advance.

Clear Aligners and Digital Smile Design

Some patients consider aligner treatment not on its own but as part of a broader aesthetic transformation. In that case, clear aligners are positioned as the orthodontic preparation step within digital smile design. The teeth are first brought to the correct position, and then whitening or veneers are applied if needed. The correct sequence is critical for both natural and lasting aesthetics.

How Is the Treatment Cost Determined?

The price of clear aligners is not fixed. Three main factors are decisive.

Case Complexity

In mild cases a 15 to 25 aligner package may be enough, while complex cases can require 40 to 60 aligners. The number of aligners directly affects the cost.

Brand Choice

There is a significant price difference between Invisalign Full and a local system. Which system is right for you should be decided based on indication, not budget alone.

Refinement and Additional Services

Some packages include refinement aligners and post-treatment retainers. Others charge separately. When comparing offers, it is important to ask about these items.

Across Turkey for 2026, clear aligner treatment ranges broadly between 37,000 TL and 160,000 TL. At Livera Clinic, a precise quote is prepared individually after the first consultation and digital scan. You can contact us and request a price evaluation for your case.

Common Concerns During Treatment

Does it hurt?

It is much more comfortable than traditional braces. A mild pressure and discomfort lasting 24 to 48 hours is normal each time a new aligner is placed. The sensation disappears after the third day.

Will it affect my speech?

There may be a slight lisp during the first week. Once the tongue adapts to the aligner, speech returns to normal. It is rare for a patient to need time off work because of this.

Are the aligners visible?

If someone looks closely, a slight reflection may be noticed. At a normal distance and in photographs, they are invisible. Attachments are made in the tooth colour.

What Happens After Treatment Ends?

The retention phase begins. Thin aligners worn at night or a wire bonded behind the lower teeth keep the teeth fixed in their new position. In the first year, every night, and in subsequent years a few nights per week, is enough. If retainers are not used, the teeth partially return to their old position within 6 to 12 months. For this reason, retention is an unskippable part of treatment.

The Limits of Clear Aligner Treatment

Like every method, clear aligners have their limits. Severe skeletal class 3 cases (lower jaw forward), severely impacted canines and complex orthodontic cases requiring multiple extractions cannot always be solved with aligners. In such cases, surgically assisted orthodontics, hybrid approaches or traditional braces deliver more predictable results. Dr. Ervin Umer will openly discuss in your consultation whether your case can be solved with aligners. We do not force aligners on a case where they are not appropriate.

The Process for International Patients

Istanbul is one of the busiest cities in the world for dental tourism. For international patients receiving clear aligner treatment, the flow is as follows. First, the suitability of the case is evaluated through an online meeting. If possible, scans or panoramic X-rays taken in the patient's home city are reviewed. The Istanbul visit is then planned. During the visit, a 3D scan is performed, the digital plan is prepared, attachments are bonded and the first aligners are handed over. When the patient returns home, they change the aligners themselves and periodic check-ups continue through online meetings. A single trip to Istanbul is often enough for the entire treatment. A short additional visit may be needed during the refinement phase. The package includes 5-star hotel accommodation and VIP transfer throughout this process.

Frequently Asked Questions

How long until clear aligners produce results?

Mild cases take 4 to 6 months, moderate cases 8 to 12 months and complex cases 12 to 18 months. The first changes are usually seen within 2 to 3 months.

Can I drink coffee, tea or red wine while wearing the aligner?

The best practice is to remove the aligner for coloured drinks. While the aligner is in, only water should be consumed. Otherwise the aligner can become stained and hygiene problems can begin.

Can I eat with the aligner in?

No, it is removed before eating. After eating, the teeth are brushed and the aligner is put back in. Eating with the aligner both breaks it and creates odour and decay due to food build-up.

What happens if I lose an aligner?

You go back to the previous aligner and inform the clinic. Most brands offer the option to produce a replacement. A new aligner is delivered within a few days.

Can I do sports with the aligner in?

Yes, the aligner even acts as a tooth protector in contact sports. Removing it is only preferred for diving or swimming.

Can I take it out if the aligner bothers me?

Being removable is an advantage, but it is critical not to fall below the daily wear time. It can be removed for short periods during important meetings or weddings.

Will there be a permanent change in my speech?

No, the first week is a slight adaptation period. After that, speech returns completely to normal.

Do teeth move at adult ages?

Yes, teeth are able to move throughout life. Because bone metabolism is slower than in children, treatment may take a little longer, but the results are satisfying.

What happens if I do not wear my retainer during the retention phase?

The teeth slowly begin to shift back toward their old position. The first 12 months after treatment are especially critical. Keeping the retainer routine is essential for the lasting success of treatment.

Is tooth extraction necessary?

It is not needed in most cases. Space is created with the IPR (enamel reshaping) technique. Extraction is only considered in very advanced crowding or skeletal discrepancy cases.

Does cigarette smoke seep into the aligner and stain it?

Yes, cigarette smoke and nicotine turn the aligner yellow. In smoking patients, the colour of each aligner is monitored. Reducing smoking during treatment benefits both the teeth and the aligners.

Can whitening be done together with aligner treatment?

Yes. In some aligner systems, whitening gel can be placed inside the aligner so that whitening happens simultaneously. This delivers a smile that is both straight and white at the end of treatment.

I have an implant, can I wear clear aligners?

An implant can be used as an anchor point, but the implant itself does not move. Planning is based on moving the neighbouring teeth. Your case must be evaluated with a clinical examination.

I have crowns or veneers, can aligners be applied?

Usually yes. In some situations the attachment bonding points may change. Planning is customised to protect the aesthetic restorations.

Conclusion: The Right Aligner at the Right Clinic

When planned correctly, clear aligner treatment is a more aesthetic, more comfortable orthodontic solution than traditional braces and interrupts your social life far less. However, success is not hidden in the brand alone but in the experience of the clinic and the doctor doing the planning. The same software and the same aligners can deliver different results in different clinics. Predicting movements, placing attachments correctly and performing IPR at the right time all depend on the clinician's knowledge and experience.

As the Livera Clinic team in Florya, we offer our patients first an honest examination, then a digitally visualised plan, and then a schedule focused on the patient rather than the brand. If you would like to find out whether clear aligner treatment is suitable for you, get in touch with us. At the first consultation we will perform a 3D scan and watch the end-of-treatment smile on screen together.
